What happens at that speed if you open the throttle more, does it die or pick up revs?
I have always set the level with no spring and fork fully compressed.

Use the insert. Put plenty of sticky grease on the tap as you thread for the insert to collect all the chips/shavings, clean...
If your going to reuse that rod then get it closed and honed back to the original size so you have the correct 'crush' on the bearing shells.
I can only say that I have had no problems with them and for the price have put five bikes back into good condition and back on the road/track.
